汽(機)車駕駛執照遺失或損毀，應如何申請換 發或補發？

1.右列身分證明文件任一(正本):國民身分證、軍人身分證、僑 民居留證明或其他有效之汽車駕駛執照。
2.損換之駕駛執照(遺失者免帶)。
3.本人最近6個月內拍攝之1吋光面素色背景脫帽五官清晰正面半 身黑白或彩色照片2張，並不得使用合成照片。
4.汽（機）車駕駛人審驗暨各項異動登記書1份。
(二)規費：駕駛執照規費新臺幣200元。
(三)注意事項：
1.如有違規、道安講習案件未結清請先結清，才可辦理補照。
2.職業駕駛人審驗到期者須同時辦理駕照審驗。
3.非本人辦理者，代辦人請攜帶國民身分證正本供查驗。
